For Colorado Residents with Disabilities
Any Colorado resident who has a total and permanent disability may obtain, free of charge, a Columbine Lifetime Fishing License. Columbine Lifetime Fishing License holders are exempt from purchasing a Habitat Stamp each year. The license will only be valid as long as the applicant maintains their Colorado residency.
What documentation do I need to get a Columbine Lifetime Fishing License?
To qualify for a Columbine Lifetime Fishing License, you must be a Colorado resident and provide one of the following as documentation of a total and permanent disability:
- A fully completed Colorado Parks and Wildlife “Physician’s Affidavit” (located on the Columbine License and Pass application), signed by a licensed physician attesting that the applicant meets the definition of a total and permanent disability; or
- A Division of Workers' Compensation "Final Admission of Liability" form indicating “Permanent Total Disability”

What is a total and permanent disability?
The Attorney General has defined a total and permanent disability as “any physical or mental impairment which prevents substantial gainful employment, but only if it is reasonably certain that such a disability will continue throughout the lifetime of the person with the disability”.
What does "CID" mean on the application?
"CID" refers to your account number with Colorado Parks and Wildlife if you have ever purchased licenses, passes, or made camping reservations in the past. If you do not know this number, or do not have an account with Colorado Parks and Wildlife, you can leave this box blank and it will be filled in for you.
How long will it take to receive my license?
Please allow at least 15 business days for verification and processing time. Less time is required for processing for applications submitted via email or in person.
Do I need to apply for the Columbine Lifetime Fishing License each year?
No, once you have been approved for the program, your license is valid as long as you remain a resident of Colorado. The license does not need to be renewed each year, and can continue to be used as long as it is intact and readable.
How can I get a replacement of my Columbine lifetime license?
Replacement licenses can be obtained in person at any of our sales locations, by email at disability.apps@state.co.us, or by leaving your information (full name, date of birth, and mailing address) at 303-291-7235.
I was already approved for the Columbine Parks Pass; do I need to reapply for the Columbine Lifetime Fishing License?
Please complete and submit the application listed above for the Columbine Lifetime Fishing License as eligibility requirements may have changed between the two programs since you were approved for the Columbine Parks Pass. If you were approved for the Columbine Parks Pass from November 2019 - Present, you do not need to reapply.
Are youth with total and permanent disabilities eligible for the Columbine Lifetime Fishing License?
Yes. Youth who meet all of the qualifications for the program are eligible to apply. However, youth are not required to hold a fishing license until they are 16 to fish in Colorado.

The rules for the Lifetime Fishing License, including qualifying documentation, are set by Colorado Revised Statute 33-4-104 and Colorado Parks and Wildlife Regulation Chapter W-0, Article II, #002.Q.2.
